Here is how you get started in our office! This is the process we take every new patient through.

Schedule in-office visit for Full Mouth X-rays (FMX) and photos. The cost of in-office FMX is $100.
- If available, you can send in existing FMX (taken within 12 months) and photos and skip the in-office visit here.
Complete the questionnaire below and submit it.
Receive your free estimated treatment evaluation, which will also include ballpark of cost, within 2-3 business days.
Schedule a full clinical exam and consultation with Dr. O'Malley.
